The fastest, lowest-risk efficiency saving available to most NHS trusts is sitting in the corridor. Here is the evidence and what to do with it.

The pressure every trust is navigating

NHS England's 2025/26 planning round has set some of the most stretching efficiency targets in a generation. A 4% productivity improvement and £11bn of savings across the system. Every trust leadership team is working through the same challenge: finding headroom in a system that has already had the obvious costs removed, without compromising the services patients depend on.

One area rarely appears in those conversations, despite already having a credible cost figure attached to it.

The number on the table

Esendex's 2024 Freedom of Information analysis across 144 trusts calculated the average annual cost of missed appointments (DNAs) at £8.2m per trust, or £1.9bn across the NHS. Trusts have used SMS reminders, patient-initiated follow-up, and the NHS App to address this. The App alone has helped prevent 1.5 million missed appointments since July 2024.

None of those interventions address one specific, documented cause: patients who arrive at the building but fail to reach their clinic room.

What the evidence shows

In NHS England's Future Connectivity Programme trial at University Hospitals of Leicester, digital wayfinding was introduced for a patient cohort where DNA rates in some groups had reached 50%. In the first month post-deployment, appointments coded as missed for 'other' reasons (the NHS category covering non-clinical, non-personal causes) fell by 19%, worth £4,320 in that month alone. The annual savings target across the full programme is £672,000.

  • Around 85% of patients ask for directions on arrival at a hospital (Deloitte Digital)
  • 30% of first-time visitors get lost
  • Staff at regional hospitals spend around 4,500 hours per year giving directions (Emory University Hospital), a cost that rarely appears on financial reporting but shows up as overtime, agency hours, and reception strain
  • St Olav's Hospital in Trondheim reported a 31% reduction in missed appointments using a comparable approach, with annual savings of around €1,500 per bed, roughly €1.5m across the 1,000-bed hospital

What the 10-Year Plan unlocks

The NHS 10-Year Health Plan commits at least 3% of annual spend to one-time service transformation investments, with a new procurement framework due in 2026/27. The plan also sets a clear 'analogue to digital' shift, yet the NHS Wayfinding Guidelines have not been updated since 2005, before the iPhone or NHS App existed.

Digital wayfinding requires no building refit. It runs on devices patients already carry, embeds into existing SMS appointment reminders, and updates in real time as wards relocate during refurbishment. UHL's trial included multilingual maps, kiosks for digital exclusion, and QR codes on appointment letters. NHS England's published case study concluded it addresses navigation barriers, improves efficiency, and reduces healthcare inequalities.

St George's University Hospitals, Oxford University Hospitals, University Hospitals of Leicester, Moorfields Eye Hospital, Blackpool Teaching Hospitals, and Stockport NHS Foundation Trust have each deployed digital wayfinding, starting with a single measurable problem, DNAs or staff hours absorbed by direction-giving.
